Urgent - possible safety issue with 2024 Rav PHEV

Today I was driving normally on the Sheffield Ring Road and came to a stop at the head of a queue at traffic lights. I was in the offside lane with a large van on my left. The lights changed after traffic had been crossing from left to right. With the lights fully on green I slowly moved forward but with limited visibility to my left because of the van. Horrifyingly two more vehicles crossed from left to right absolutely at red!  Then as I began to accelerate my power died. The accelerator totally failed to respond. There was a big queue of traffic behind me and traffic in the near side lane was beginning to flow. I looked at the display but could see no obvious reason for the loss of power. I did have three repeated bleeps. Fortunately the cars kept moving forward at 10mph and putting my hazards on I found a slot, moved left and turned off the dual carriageway into a side road.  I could see no way whatsoever of getting back to “normal”. I pressed the start button to off and everything returned to normal when I restarted. As I drove away I thought it possible that a failsafe collision avoidance system had taken over….but in fact it created danger if that was the reason. Talked to Vertu Toyota and tomorrow am going in for a diagnostic check but the man on the phone could only offer the same theory. If so it is ludicrous and we should be demanding a recall.  I was in much more danger by being stranded in the middle of a junction than merely being annoyed at someone jumping a red light.   My dash am has the exterior view of the event but sadly there is no way of recording what came up on the multi information screen in front of me.  If there was a visible warning I certainly couldn’t see it. All your theories and experiences welcomed. Should I, COULD I have moved from D to N and restarted whilst wheels were turning. I surely could not have just switched off.  What choices did I have? None it seems.

You can report a potential safety issue  to DVSA, and if there are a sufficient number of similar reports, the DVSA will investigate the issue with the manufacturer - see:

That is exactly why my Dashcam is between the two seats on the left side of my head when I drive. It solves disputes like "you didn't indicate" and other such stories from idiots.

I used to have exactly the same issue with my Peugeot 3008 GT300 hybrid4 PHEV. Random total loss of power for several seconds when accelerating from standstill. Dangerous as hell at junctions and roundabouts. They never got to the bottom of it. Could have been faulty sensors, faulty software, or anything really. Several occasions it happened there were no other vehicles in front or behind.

The diagnostic check showed no other likely cause than the PCA system "doing its job". I have now set the PCA at lowest level but the dealers and myself will be writing to Toyota to put this on the record. My view is it is silly, verging on stupid and urgently needs a mod to allow the OK button to release the speed lock.    Incidentally I was not amused to find that the offer that provoked me into buying the Rav Phev "has been extended" until June 30th with slightly amended terms but still offering a British Gas Hive installation.  Think very carefully ! There is apparently some alternative on offer, i.e. £250 charging credit on the Toyota charging network. But they won't backdate it.

I just hope this kind of stupid stuff is sorted before I get my replacement R4P in September 🤣. Hopefully you've made a preemptive difference to my miserable life. 👍🙏
